Oral peptides: do peptide pills work?

The short answer

Most peptides that need to reach the bloodstream do not work well when swallowed, because digestion breaks them apart. A few FDA-approved medicines get through, such as Rybelsus and Wegovy tablets and Mycapssa capsules. Others, such as Linzess, work inside the gut. Supplement and research-labeled "peptide pills" are different products, and that evidence does not carry over.

Three different things get called peptide pills.

A peptide is a short chain of amino acids. Amino acids are the small building blocks that also make proteins. The one question answered here is what happens when a peptide is swallowed.

The words peptide pill can describe three very different products. The first is an FDA-approved medicine made to be taken by mouth. The second is a food supplement, such as collagen peptides. The third is a capsule of an unapproved peptide, often sold with a research label. Evidence for one group does not apply to the others.

This page is general education. It gives no amounts, no timing, no product picks, and no seller information. It was not independently medically reviewed. Questions about your own treatment belong with a clinician.

Why swallowing a peptide usually fails.

The gut is built to break food proteins into small pieces. A peptide medicine meets the same system. A 2022 review in Theranostics describes the barriers in order. Stomach acid and the enzyme pepsin break down most protein and peptide drugs in the stomach. More enzymes, such as trypsin, act in the small intestine.

A peptide that survives still has to cross the gut wall. The review describes a mucus layer first. Below it sits a layer of cells joined by tight seals. The gut cells can also break down a drug or pump it back out. Each step lowers how much intact peptide reaches the blood.

Bioavailability is the share of a dose that reaches the bloodstream unchanged. An injection under the skin skips the stomach and gut wall. A swallowed peptide faces all of them. So a peptide with good bioavailability as a shot can have very low bioavailability as a pill.

FDA-approved oral peptide medicines and how they work.

FDA-approved oral peptide medicines come in two kinds. Some are built to get the peptide into the blood. Others are meant to stay in the gut and act on the gut lining. The products below were checked against their current FDA labels. They are examples. The list does not include every approval.

  • Rybelsus and Ozempic tablets (semaglutide, NDA 213051). Ozempic tablets share Rybelsus's label and are not the Ozempic shot. Use and people covered: with diet and exercise, blood sugar control and lower heart-event risk in defined adults with type 2 diabetes. How it enters the body: an absorption enhancer called SNAC, with absorption mainly in the stomach.
  • Wegovy tablets (semaglutide, NDA 218316). Use and people covered: with a reduced-calorie diet and more physical activity, long-term weight reduction in defined adults. They also lower the risk of major heart events in adults with established heart or blood vessel disease and obesity or overweight. How it enters the body: the same SNAC design, with absorption mainly in the stomach.
  • Mycapssa (octreotide, NDA 208232). Use and people covered: long-term maintenance treatment of acromegaly (too much growth hormone). It covers people who responded to and tolerated octreotide or lanreotide. How it enters the body: a coated capsule stays closed past the stomach. The label reports that the gut lining let substances pass more easily for a short time. This was seen 2 hours after a dose and was back to normal by 5.5 hours.
  • Acting inside the gut: Linzess (linaclotide, NDA 202811) and Trulance (plecanatide, NDA 208745). Use and people covered: constipation conditions named on each label, including irritable bowel syndrome with constipation and chronic idiopathic constipation, in defined patients. How they work: each acts on the gut lining. Both labels say the drug is minimally absorbed with negligible systemic availability after oral use.

How the semaglutide tablet design works, and why it stays with that tablet.

A 2018 study in Science Translational Medicine looked at how oral semaglutide gets absorbed. It combined studies in people and in dogs. The summary does not separate which findings came from which. Absorption took place in the stomach, in a small area close to the tablet surface. It required SNAC in the same tablet.

The study found that SNAC buffers the area around the tablet, which protects semaglutide from enzymes. It raises absorption only for a short time. The authors described the mechanism as compound specific. That means the finding applies to this drug in this formula. It does not show that SNAC helps any other peptide.

Only a small share of each dose reaches the blood. The shared label estimates bioavailability at about 0.4% to 1% for Rybelsus and about 1% to 2% for Ozempic tablets. The label also says the two tablets are not substitutable milligram for milligram. In plain words, a milligram of one does not equal a milligram of the other.

The Wegovy label estimates about 1% to 2% for its tablets. The labels also set strict rules about food, water, and timing, because those change absorption. Those rules stay on the labels and are not repeated here as instructions.

What the PIONEER 1 trial measured in people.

PIONEER 1 was a 26-week trial of oral semaglutide against a look-alike pill with no drug (placebo). It enrolled 703 adults with type 2 diabetes not controlled by diet and exercise alone. Neither patients nor researchers knew who got which pill.

The main result was HbA1c, a blood test of average blood sugar over recent months. All three tablet strengths lowered it 0.6 to 1.1 percentage points more than placebo in the main analysis. In that analysis, only the highest strength showed a clear weight difference: 2.3 kg more than placebo.

Stomach and gut side effects were the most common. They were mild to moderate and faded over time. Several authors worked for Novo Nordisk, which makes the product. The trial does not show results for other people, other uses, or any other oral peptide.

Oral and injected peptides: what actually differs.

An oral and an injected product can share one ingredient and still be different products. They have separate applications, labels, uses, and studies. The differences below matter for the oral question.

Neither route is better for everyone. The right product for a person depends on the diagnosis, the approved uses, other medicines, and a clinician's judgment. This page cannot make that call. Injected products carry their own questions about sterility, device use, and quality.

  • How much reaches the blood. The Wegovy label estimates about 89% bioavailability for its injection under the skin. It estimates about 1% to 2% for its tablets.
  • Amounts are not interchangeable. Because so little of a tablet is absorbed, milligram numbers for pills and shots cannot be compared. Even Rybelsus and Ozempic tablets, with the same drug and label, are not substitutable milligram for milligram.
  • The same holds for octreotide. In healthy volunteers, the Mycapssa label reports that a far larger oral amount gave total exposure similar to a much smaller injection.
  • Label rules. Tablet labels set conditions about food, water, and other medicines. Injection labels set their own rules for handling and use.
  • People studied. Each product's approval covers the people and uses in its own label. An approved use for one form does not transfer to the other.

Collagen peptide supplements and what a blood level shows.

Collagen peptides are a common kind of peptide pill. They are food supplements made by breaking collagen into smaller pieces. They are not FDA-approved medicines. Some tiny collagen fragments do reach the blood after they are swallowed.

A 2005 study gave healthy volunteers broken-down gelatin, a collagen product, after a 12-hour fast. A peptide form of hydroxyproline, a collagen building block, rose in the blood. It peaked 1 to 2 hours after intake. It fell to about half that level by 4 hours. The main fragment found was Pro-Hyp, a piece just two amino acids long.

That study measured blood levels only. It did not measure skin, joints, or any other health result. A fragment showing up in blood does not prove it does anything useful.

Oral forms of unapproved peptides, such as oral BPC-157.

Some sellers offer capsules or liquids of unapproved peptides, such as BPC-157. FDA says BPC-157 is not part of any FDA-approved drug. So no approved label sets out how an oral form is absorbed, what it treats, or who it is for.

In 2026, FDA staff reviewed BPC-157 for compounding, which is when a pharmacy prepares a drug for one patient. It covered several forms, including oral capsules, for ulcerative colitis. The briefing says: "We found no human pharmacokinetic data for BPC-157 after oral, SC, nasal, or transdermal administration." Pharmacokinetic data show how the body absorbs and clears a drug.

On July 23, 2026, an FDA advisory committee voted 8 to 6, with one abstention, in favor. It recommended adding BPC-157 to the list of substances pharmacies may use. That vote was nonbinding. It did not approve a BPC-157 drug or create an FDA label.

The briefing also notes an older lab report that BPC-157 resisted breakdown in stomach fluid in a test tube. A test-tube result does not prove that people absorb it. Cell and animal findings describe cells and animals. Results in people need studies in people.

How to check whether an oral peptide claim fits a product.

Use four questions before you trust a claim that a peptide works by mouth. Answer each one for the exact product in front of you. The ingredient name alone does not answer them. Write down the product name, the claim, and where each answer came from.

The same questions apply to under-the-tongue (sublingual) peptide claims. No approved sublingual peptide product is covered on this page. Each such product would need its own studies in people.

  • Was this exact pill or capsule tested in people? A study of a different product, an injection, or animals does not count.
  • Does it use the same absorption design the study used? For semaglutide tablets that means the SNAC formula. Another capsule with the same ingredient name may not have it.
  • Did the study measure a health result, or only a blood level? A peptide in the blood does not prove a benefit.
  • Is the product FDA-approved for that use? Approval covers one product, one use, and defined people.

Apply the four questions to a capsule sold as oral BPC-157 with a research-use label. First, FDA's 2026 briefing found no human data on how BPC-157 is absorbed after oral use. Second, no study shows the capsule uses a proven absorption design. Third, no human health result is established. Fourth, no BPC-157 drug is FDA-approved.

Every answer is no or unknown, so the claim is not established for that product. Compare a Rybelsus tablet for blood sugar in an adult with type 2 diabetes. That tablet, with SNAC, was tested for HbA1c in people and is approved for that use.

When an answer is unknown, you can still act. Look up the name in an approval record. Look for a published human study of that exact product. Or bring the four answers to a clinician or pharmacist.

Common questions

Do oral peptides actually work?

Most peptides that need to reach the blood do not work well when swallowed. Stomach acid, enzymes, and the gut wall stop most of them. A few FDA-approved products, such as semaglutide tablets and Mycapssa capsules, are designed to get through. That evidence applies only to those exact products and their approved uses.

What peptides can be taken orally?

FDA-approved examples include Rybelsus, Ozempic tablets, and Wegovy tablets (semaglutide) and Mycapssa capsules (octreotide), which reach the blood. Linzess and Trulance also work by mouth but act inside the gut. Collagen supplements and research-labeled capsules are not approved medicines, even when they share a peptide name.

What do oral peptides do?

It depends on the exact product. Semaglutide tablets reach the blood and are approved for blood sugar control, heart risk, or weight reduction in defined adults. Linzess and Trulance stay in the gut and are approved for certain constipation conditions. Supplement and research-labeled peptide pills have no FDA review of whether they work.

Can you take BPC-157 orally?

No FDA-approved BPC-157 drug exists, oral or otherwise. FDA's 2026 briefing found no human data on how BPC-157 is absorbed after oral use. An advisory committee's 8 to 6 vote to recommend it for pharmacy compounding was nonbinding and did not approve a drug.

Is it better to inject peptides or take them orally?

There is no general answer. Oral and injected forms are separate products with separate labels, uses, and studies. For semaglutide, the Wegovy label estimates about 89% bioavailability for injection and about 1% to 2% for tablets. The right product for one person is a decision for a clinician.
