Do not include medical records, treatment details, or other sensitive personal information. Submissions are private until an editor verifies and intentionally publishes a correction.

02 · Triage standard

What happens next

Material corrections are dated and preserve an auditable before-and-after record.

  • We acknowledge valid submissions with a private reference.
  • An editor checks the exact page, wording, source, scope, and current primary record.
  • Medical or regulatory interpretations require qualified review before publication.
  • Material corrections preserve the prior wording, revised wording, date, and sources.
  • Rejected requests are not published merely because they were submitted.
  • Private intake records expire within 90 days and can be deleted sooner from the confirmation screen.
